Doctors explain how to detect silent hypoxia (2021/06/10)

Doctors say that a dangerous COVID symptom called silent hypoxia can be easily detected at home. People with silent hypoxia feel fine, even as their blood oxygen saturation drops and compromises organ function, potentially causing death. In recent weeks, there's been an increase in sudden COVID deaths that experts have linked to undetected hypoxia. Doctors say it's important for COVID patients to test their saturation levels regularly, using a pulse oximeter. A reading below 95% could mean it's time to visit the hospital.

The virus continues to spread, as does the number of sudden deaths from COVID. Experts believe that in most of these cases, the victim was suffering from silent hypoxia. That's when the body loses oxygen, but there are no signs of a problem initially. The patient has no trouble breathing, until they deteriorate rapidly without warning. The condition can only be detected with a device called a pulse oximeter.

Generally speaking, normal blood oxygen saturation is in the ballpark of 97, 98, 99%. If it drops to 95 or 96%, that's not a reason to be too anxious, because you might have been talking for too long, or you might be out of breath after exercise. You can take a few deep breaths and see if your levels slowly go up. If they do, you should be all right. But if you test yourself and your blood oxygen is 94% or below, and it doesn't get higher even after you adjust your state, then I would suggest you go to a hospital as soon as possible.

Wu Chang-teng, a pediatric emergency physician at Linkou's Chang Gung Memorial Hospital, says people with silent hypoxia may eventually start experiencing symptoms that prompt a hospital visit. Four common symptoms are chest tightness, extreme difficulty breathing, shortness of breath, and taking breaths at least three times more often than normal. Other warning signs are lips and fingers that turn blue due to oxygen deprivation.

They may already have obvious signs like skin and lips that have turned blue. Or their face has turned pale, or they're experiencing wheezing, chest tightness, or low blood pressure. Usually when these signs manifest, the patient's hypoxia is in the symptomatic stage, and it's time to go to a hospital right away.

Physicians warn that silent hypoxia can kill. On Wednesday, the Central Epidemic Command Center sent 15,000 pulse oximeters to COVID-positive patients nationwide, so that they can monitor their condition at home and get timely medical care.

猝死案例不斷增加,指揮中心研判,可能和隱形缺氧有關,醫師吳昌騰提醒,患者初期沒異狀,但內部器官已快速受損,可觀察手指發青、無法深呼吸、胸悶、換氣快3倍來自我研判。但醫師柳朋馳則認為,正因初期沒症狀,最好使用血氧機,如果血氧濃度掉到92%到94%,就要趕快送醫。

疫情延燒,到院前猝死案例,不斷增加,分析大多和隱形缺氧有關,不過,醫生表示,隱形缺氧初期沒有症狀,也不會呼吸急促,病情卻會快速惡化,得靠血氧機,才測得出來。

“一般大部分,正常人的血氧濃度,最常見在97% 98% 99%三個範圍在走,假如偶爾掉到95% 96%也不用太緊張,你可能處在講話講太久,或是可能剛運動完有點喘氣,其實可以做幾下深呼吸,看起來血氧慢慢升上去,原則上都沒有什麼大問題,假如說量起來血氧濃度在94%以下,原則上假如做這些改變,還沒辦法看到顯著性上升,建議你可能儘快要到醫療院所”

如果血氧下降到94%,沒有回升的跡象,就得趕快送醫,要是掉到92%,可能就需要氧氣設備。

不過,林口長庚兒科急診醫師吳昌騰提醒,這類的病患,通常就醫前2至7天前會出現四個症狀,像是胸悶、無法呼吸,呼吸急促,在正常呼吸速率的3倍,或是嘴唇、手指發青,就要提高警覺。

“已經有明顯皮膚、嘴唇都已經發青,或者臉色蒼白或者會喘,會胸悶,血壓會掉,這種表現的時候,大部分都是相對有症狀性的缺氧,這時候要儘快送醫院”

隱形缺氧成為殺手,指揮中心也在9日,配送1.5萬台血氧機,給快篩陽性患者帶回家使用,就是希望能降低猝死案例。
